Every growing med spa eventually faces the same crossroads: hire someone in-house to run marketing, or bring in an outside team that already knows the industry. There’s no universal right answer, but there is a right answer for your clinic, your growth stage, and your budget. Understanding what each path actually costs — in time, money, and results — makes the decision a lot easier.

Weighing the Two Options

An in-house hire offers something an outside team can’t: someone in the building every day who knows your staff, your treatment menu, and your patients by name. But one person, or even a small internal team, rarely has expertise across paid ads, SEO, email, content, and analytics all at once. Med spas that go this route often end up paying a full-time salary and benefits for a generalist who’s strong in one channel and guessing at the rest.

An agency flips that trade-off. You get a full bench of specialists — someone dedicated to paid media, someone else to SEO, another to content and email — for a fraction of what it would cost to hire that same team internally. The tradeoff is that they aren’t sitting in your break room, so clear communication and reporting matter more than ever.

Cost is usually the deciding factor, but it shouldn’t be the only one. Before you commit either way, it helps to see what real med spa marketing looks like in practice — the systems, reporting, and communication that separate a marketing partner who moves the needle from one that just sends invoices. That’s the real benchmark, whether you’re evaluating an agency or building a team from scratch.

For most single-location med spas that are still scaling, an agency delivers more expertise per dollar and gets campaigns live faster. Larger, multi-location groups with the budget to build a full internal team may eventually outgrow the agency model — but even then, many keep a specialized partner on retainer for the channels that change too fast to manage part-time. The best approach is the one that matches where your clinic is right now, not where you hope to be in five years.
